CHI Memorial now offers an online patient record request tool, which is easy to use and free. You'll need to verify your identity by uploading a photo of your driver's license, which you can take with your phone or computer.

The Medical Records office is located on the first floor of CHI Memorial Hospital Chattanooga and is open Monday-Friday, 8 a.m.-4:30 p.m. You'll usually get your records within 7 to 10 business days.

Patients of CHI Memorial Medical Group may request medical records by contacting their practice to request a medical release form. A signed, completed release form is required. A fee may apply.

To access records in your patient portal, message our medical records team through the portal. You can also request records through MediCopy, our trusted partner for information management.

Authorization to release information

If a physician’s office is requesting your records, please have them fax our medical records department at (423) 697-2320. For requests involving FMLA or short-term disability forms or records, print and complete the Authorization to Release Information form, and send the completed form back to us via mail or fax.
