New CommonSpirit Health patient portal and app

We are introducing our new CommonSpirit Health patient portal and app, designed to deliver the same secure, convenient access you're used to, with a refreshed look and a more seamless digital experience.

  • Starting June 28, 2025, you may receive texts or emails to help you activate the patient portal or download the CommonSpirit - KY/TN/GA/TX app from Google Play or the Apple App Store. 

  • If you are a new MyChart user, our staff can help you activate your MyChart account during check-in.

  • To guarantee you access to all features, please update your bookmarks or favorites to our new web address: patientportalsouth.commonspirit.org.

It’s free and available for patients at the following hospitals and medical groups.

Create a new account

Your current portal login won’t carry over to the new CommonSpirit patient portal. To access the CommonSpirit patient portal, you must create a new account here.

  • After creating an account, you will receive an activation email

  • Follow the link in the email and use your information to verify your identity

  • After verifying your identity, you can create a user name and password

Access your new account 

  • Appointments, prescriptions and messaging will not become available until after your next visit with your provider. 

  • During your next visit, your care team will make sure your information is accurately reconciled and help you set up your CommonSpirit patient portal account if you have not already done so. 

For My OneCare users

You will continue to view and access My OneCare (powered by FollowMyHealth) during this transition, but you will no longer be able to message your doctor, refill prescriptions or request appointments after June 28. 

  • Contact your doctor’s office or pharmacy to refill prescriptions. 

  • Call your doctor's office or request an appointment through our website.

Our NextGen patient portal gives our heart clinic patients instant access to appointments, test results, refill requests and more—all in one place. It’s the only tool that lets you view lab, imaging, and test results that don’t require any changes to your care plan.

Getting started

To create an account, contact the portal coordinator by calling (423) 697-2338. Leave a voicemail with your name, date of birth and email address. Once your account is set up, you’ll receive an invitation to join NextGen at the email you provided during your visit. This invitation will come from noreply@medfusion and include instructions to complete your registration. If you don’t see the email, please check your spam folder.
