That’s why we’re proud to offer secure online access to your health information, making it easier and more convenient to manage your care.
Open communication with your health care provider ensures you receive the very best possible care. Follow these tips during your next appointment:
Bring a complete list of your medications, including over-the-counter drugs, vitamins and herbal supplements, along with their dosages and directions.
Have a list of health concerns you’d like to discuss with your provider. Review your questions for your provider and ensure your top three concerns are addressed during your visit.
Bring details of any health care providers you’ve previously seen, including names, dates of treatment and contact information.
Please arrive 30 minutes early to ensure timely check-in and no more than 10 minutes late to help minimize wait times. We understand emergencies may arise and will accommodate them when possible.
Only urgent issues will be addressed after hours. For non-emergent concerns, please schedule an appointment. Prescription refills are limited outside regular office hours.

We accept most major insurance plans. All insurance copays, coinsurance and deductibles are due at the time of service. Please be prepared to settle these before you leave the office.
To ensure accurate billing and timely communication, please inform our office of any changes to your address, phone number or insurance information. This helps us file claims correctly and avoid unnecessary billing issues.

The Office of Disease Prevention and Health Promotion recommends you visit your primary care provider annually for a wellness checkup. However, you should schedule additional appointments as needed for new symptoms, follow-ups or managing chronic conditions.

To obtain your medical records, you must complete a medical authorization form. Call our office for more information.
