CommonSpirit Patient Portal FAQs

In 2025, we launched the CommonSpirit Patient Portal and Mobile App for CommonSpirit - Memorial Hospital patients.

On February 1, we're excited to introduce the CommonSpirit Patient Portal and Mobile App for patients at The Chattanooga Heart Institute, unifying our CommonSpirit locations and simplifying how you access your health information and manage your care across Tennessee and Georgia.

The patient portal offers secure, convenient access to your health information with a refreshed look that reflects the association of CommonSpirit - Memorial Hospital and The Chattanooga Heart Institute with CommonSpirit Health. 

The questions and answers below are meant to help make this transition as seamless as possible for you.


 

What is the CommonSpirit Patient Portal?

The CommonSpirit Patient Portal is our new, secure online tool that lets you take charge of your health — anytime, anywhere. You can view test results, message your care team, pay bills and more. You can access the portal from your computer or download the CommonSpirit App on your phone from the App Store (iPhone) or Google Play (Android).

Why are CommonSpirit - Memorial Hospital and The Chattanooga Heart Institute making this change?

We know the health care environment is complicated — but your experience with your health information doesn't have to be. This tool is designed to simplify and improve your experience through one single, secure destination where you can manage your health online. This new platform is designed to be intuitive and user-friendly, making it easier than ever to view test results, communicate with your care team and more.

Do I need to create a new account?

  • If you already have the CommonSpirit Patient Portal or App: 

    • You can simply use your current login information to access the CommonSpirit Patient Portal, which offers the same secure, convenient access to your health information that you’re used to. 

    • Your health information and records from CommonSpirit - Memorial Hospital and The Chattanooga Heart Institute will automatically transfer to the CommonSpirit Patient Portal and be available following your next appointment.

  • If you currently use MyChart, you can use your existing username and password when logging into the new patient portal. There’s no need to set up a new account.

  • If you currently use the FollowMyHealth portal, you still have access to view your past health information in FollowMyHealth, but all other tools and features within the portal are no longer available. Please sign up for a new account with the CommonSpirit Patient Portal here. The CommonSpirit Patient Portal will guide you through the process of setting up your username and password.

  • If you currently use the NextGen portal: 

    • Your NextGen portal login information will not work with the CommonSpirit Patient Portal and Mobile App. You will need to create a new account on or after February 1 if you don’t have a CommonSpirit Patient Portal account already. 

    • After activating your account, your health information will automatically transfer and be available following your next appointment.

  • If you have never used a patient portal, you will need to create a new account. Please sign up for a new account with the CommonSpirit Patient Portal here. The CommonSpirit Patient Portal will guide you through the process of setting up your username and password.

What does this mean for my medical records and health information?

We are committed to ensuring you have continued access to your health information as we make this transition to the new patient portal. Here’s what you need to know:

  • Your health information from CommonSpirit - Memorial Hospital hospital visits or hospital stays has automatically transferred to the new patient portal.
  • Your health information from Memorial’s doctor’s offices and clinic visits, including The Chattanooga Heart Institute, will be available in the new CommonSpirit Patient Portal following your next appointment on or after February 1.
  • In the meantime, your doctor still has full access to your records, and you can call the doctor’s office for help managing appointments, test results or other questions. 

What can I do with the new portal?

You can:

  • Manage appointments with established providers 
  • Complete pre-check-in tasks
  • View test results and After-Visit Summaries
  • Communicate securely with your care team
  • Request prescription refills
  • Pay bills online
  • Complete post-visit questionnaires
  • Use our expanded Find a Doctor & Find a Location functionality

Is the portal safe and secure?

Yes. We take your privacy very seriously. The CommonSpirit Patient Portal uses industry-standard technology and security measures to protect your personal health information. Only you and your authorized caregivers can access your account, and all data is encrypted to prevent unauthorized access. 

Where can I download the mobile app?

You can download the CommonSpirit App for free from the App Store (for iPhone) or on Google Play (for Android). Search for “CommonSpirit – AR/GA/KY/TN/TX” to find the app quickly.

Will my provider or care team change?

No. Your care team remains the same care team you know and trust. The new portal is simply a better way to stay connected with them.

What if I don’t currently use the CommonSpirit Patient Portal? 

If you don’t currently have a CommonSpirit Patient Portal account, we encourage you to sign up. This patient portal provides a convenient, secure way for you to manage your health, either online at https://patientportalsouth.commonspirit.org/PRD/ or through our mobile app. You’ll be able to use the portal and app to access test results, send messages to your provider, pay bills and more.

What if I don't have a computer or smartphone, or I'm not comfortable using technology?

We understand that not everyone has access to technology or feels comfortable using it. You can still access your health information and manage your care by contacting your provider's office directly. You can request a printed copy of your After-Visit Summary and complete any necessary questionnaires over the phone. We are committed to ensuring that everyone has access to the care they need, regardless of their technological abilities and preferences.

Why am I receiving messages and notifications from CommonSpirit Health?

To ensure a unified experience across all of your care, the patient portal reflects the CommonSpirit brand. This means some of your messages and notifications, such as important reminders and test results, may now come from CommonSpirit. Rest assured, these messages are from the same physicians and caregivers you’ve always relied on.

What happens to my FollowMyHealth Account?

You still have access to view your health information in FollowMyHealth, but all other tools and features within the portal are no longer available. Your past health information in FollowMyHealth will be available in the new CommonSpirit Patient Portal following your next appointment.

Will I need to set up a new account with the new CommonSpirit Patient Portal?

Yes, you will need to make a new account for the CommonSpirit Patient Portal. Then, after your next visit on or after February 1 with your provider, your past health information will be transferred to your new CommonSpirit Patient Portal account.

What if I need help managing my care before my next appointment with my provider?

Your doctor still has full access to your records and health information. You can always call your doctor’s office for help managing appointments, test results or other questions. For prescription refills, we recommend calling your pharmacy.

If I am using the CommonSpirit - Memorial Hospital MyChart portal today, do I need to create a new account for the new portal?

No. You will use the same username and password you use today to log into the CommonSpirit Patient Portal. Your health information from your hospital visits will be transferred seamlessly to the new patient portal. Your health information from Memorial doctor’s offices and clinic visits will be available in the new CommonSpirit Patient Portal following your next appointment.

How is the new CommonSpirit Patient Portal different from the MyChart portal I use today?

The new CommonSpirit Patient Portal offers the same secure, convenient access to your health information that you’re used to. The portal will have a refreshed look that reflects Memorial’s association with CommonSpirit Health.

What is changing about how I manage my health information?

Your medical records have been seamlessly transferred to the new CommonSpirit Patient Portal. Any new updates or messages from your doctor will appear in the new patient portal.

Can I continue using the standard MyChart app?

Yes. However, for a more seamless experience, we recommend you remove the older app and install the new CommonSpirit Patient Portal Mobile App. 

To uninstall your older app:

To remove on iPhone:

  • Find the MyChart icon on your home screen

  • Tap and hold the app icon until a menu appears

  • Select Remove App

  • Tap Delete App to confirm

To remove on Android: 

  • Open your app drawer and locate MyChart

  • Tap and hold the app icon

  • Drag it to the Uninstall option that appears or tap Uninstall in the menu

  • Confirm the uninstallation

How do I uninstall previous patient portal apps?

To remove on iPhone:

  • Find the MyChart or My OneCare icon on your home screen

  • Tap and hold the app icon until a menu appears

  • Select Remove App

  • Tap Delete App to confirm.

To remove on Android: 

  • Open your app drawer and locate MyChart or My OneCare 

  • Tap and hold the app icon

  • Drag it to the Uninstall option that appears or tap Uninstall in the menu

  • Confirm the uninstallation

Can I still use my NextGen patient portal today?

Yes. You will continue to have access to your historical health information after February 1. If you already have a CommonSpirit Patient Portal to manage your health information, your health information will immediately transfer over to the CommonSpirit Patient Portal following your first appointment on or after February 1. 

What happens to my health information in the NextGen portal after February 1?

After February 1, you will still be able to view your historical health information within the NextGen portal. We encourage you to register for a new account in the CommonSpirit Patient Portal when it becomes available on February 1.

Will I need to set up a new account with the CommonSpirit Patient Portal if I am a new patient?

Yes. After February 1, you will need to make a new account for the CommonSpirit Patient Portal. Your health information will transfer seamlessly and be available following your first appointment on or after February 1.

How do I access my health information like appointments, prescriptions and bills after February 1?

Your past health information will continue to be available to access within NextGen. Once you make a new account in the CommonSpirit Patient Portal, your health information will be transferred there and available following your first appointment on or after February 1. 

What if I need help managing my care between February 1 and my next appointment with my provider? 

Your doctor still has full access to your records and health information. You can always call your doctor’s office for help managing appointments, test results or other questions. For prescription refills, we recommend calling your pharmacy.

How is the new CommonSpirit Patient Portal different from the NextGen portal I use today?

The CommonSpirit Patient Portal offers the same secure, convenient access to your health information that you’re used to. The portal will have a refreshed look that reflects The Chattanooga Heart Institute’s association with CommonSpirit Health. 

Checking in for appointments

Can I check in for appointments through the patient portal or mobile app?

Yes. When you access the patient portal through the mobile app or your web browser, you will see an option to check in for an upcoming appointment. You may also receive notifications about pre-check-in tasks through the patient portal and/or via text message, depending on the communication preferences you have selected in your patient portal settings. Some of these messages and notifications may now come from CommonSpirit. Rest assured, these messages are from the same physicians and caregivers you’ve always relied on. Please note that you may still be asked to check in and present your insurance information when arriving for an appointment. 

What kind of information will I need to provide during the pre-check-in process?

You may be asked to verify your insurance information, update your medication list, complete health questionnaires and review/sign consent forms during the pre-check-in process. Please note that you may still be asked to check in when arriving for an appointment. 

Is it required to complete pre-check-in tasks before my appointment?

While it's not required, completing pre-check-in tasks before your appointment will save you time at the office and allow us to better prepare for your visit. If you don't have time to complete pre-check-in tasks before your appointment, don't worry. You can still complete the necessary steps upon arrival. 

What happens when I arrive for my appointment?

You can check in at the front desk or use one of our convenient kiosks, if available. Our staff will be available to assist you with the check-in process.

Accessing my After-Visit Summary

How do I access my After-Visit Summary (AVS) after my appointment?

You will receive a notification when your After-Visit Summary is available in the CommonSpirit Patient Portal and Mobile App. Log in and select "View After-Visit Summary" under your most recent appointment. If you don't see your AVS in the portal right after your visit, please check back later, as it may take some time for the AVS to be processed and uploaded. If it's still not available after a reasonable amount of time, contact your provider's office. Please note that some of these messages and notifications may now come from CommonSpirit. Rest assured, these messages are from the same physicians and caregivers you’ve always relied on.

What if I have questions about the information in my After-Visit Summary (AVS)? 

Non-urgent medical questions may be submitted through the patient portal or by contacting your clinic or care facility directly. If you need an immediate answer or have an urgent request, please call your clinic or care facility.

Will I receive any questionnaires after my visit?

You may receive a notification about a new questionnaire related to your visit or hospital stay in the patient portal and mobile app. Please complete the questionnaire to help us improve our services.

Messaging my provider      

When can I message my provider? 

For Memorial clinics and The Chattanooga Heart Institute Patients: Once you have completed your first appointment visit using the new patient portal, you will be able to message your providers directly. To ensure you are able to receive messages electronically, please complete the pre-registration form at the time of your scheduled appointment.

For CommonSpirit - Memorial Hospital Patients: Our acute care providers, such as those in our emergency department, urgent care, and inpatient hospital, do not utilize the patient portal as a primary communication tool. After discharge, you will most likely be referred back to your primary care provider, who will use secure messaging. 

Managing my prescriptions

Can I use the portal to view my prescribed medications? 

Yes. Once you have created an account, medications from CommonSpirit - Memorial Hospital and The Chattanooga Heart Institute will automatically transfer to the CommonSpirit Patient Portal after your next appointment. 

Can I request a refill of my prescription through the portal or mobile app? 

Yes. Once your prescriptions appear in the CommonSpirit Patient Portal, you can request refills. In the meantime, please contact your pharmacy to refill a prescription.

Managing a bill

How do I pay an outstanding bill on the CommonSpirit Patient Portal?

You can log into your CommonSpirit Patient Portal online or through the CommonSpirit Mobile App. Follow the online instructions for paying your bill:

For CommonSpirit - Memorial Hospital visits or stays, you can log into your CommonSpirit Patient Portal online or through the CommonSpirit Mobile App. Follow the online instructions for paying your bill:

  • Navigate to the View Bill option.

  • Once you click on View/Pay Bill, your account information will be displayed, including open visits with balances, total balance owed and itemized and monthly statements. 

For Memorial clinics or doctor’s offices, bills for services provided after June 28, 2025 can be paid using the new CommonSpirit Patient Portal online or through the CommonSpirit App. For bills for services prior to June 28, 2025, please reach out as follows regarding bills:

  • For Memorial Medical Group, call 833-513-3903.

  • For The Chattanooga Heart Institute, call your doctor’s office directly.

  • For Memorial Outreach Lab, click here.
