Care coordinators have special training to help you manage your health care. They are also called care managers or health navigators. Your doctor might suggest a care coordinator if you have a complicated health problem.

Care coordinators can be nurses. They can also be social workers. They work with your doctors and your care team. This is to make sure that everyone has the same information about your care.

They may also:

  • Keep track of your health conditions.
  • Help you keep track of health appointments, tests, and medicines.
  • Connect you with support services. These are things like housing and transportation.
