St. Luke’s Health – Memorial Livingston is a modern, innovative hospital dedicated to providing compassionate, high-quality care 24/7. Conveniently located off Highway 59 South, this 66-bed facility delivers more than 60,000 patient services annually—half through its Level IV trauma-designated emergency department. Patients have access to a wide range of services including orthopedic care, women’s health and a state-of-the-art sleep center. Advanced imaging services include MRI, CT, 3D mammography, 4D ultrasound, cardiac echo and more—all close to home.

Frequently asked questions

St. Luke’s Memorial offers an online patient record request tool, which is easy to use and free. If you prefer to pick up your medical records in person, you must complete a medical authorization form and bring it or mail it to St. Luke's Memorial – Livingston.

Birth certificates

A birth certificate is issued for all infants born at St. Luke's Health – Memorial and sent to the State of Texas.

If the father’s name will be included on the birth certificate, an Acknowledgement of Paternity (AOP) form must be completed during the baby’s hospital stay or within the first 3 days of life. AOP forms can be completed Monday through Friday, between 8 a.m. and 3 p.m.

You can preregister at the hospital admissions desk or by phone. During preadmission, you’ll need to provide the following:

  • Insurance information
  • Date of birth
  • Social security number
  • Phone number
  • Mailing address

Please schedule a preadmission appointment for required tests (like blood work, EKG or X-rays) and to meet with a nurse for health history, surgical consent and pre/post-op education. You may also consult with an anesthesiologist.

Bring the following to your appointment:

  • Driver's license or photo ID
  • Social security card or number
  • Insurance cards
  • Physician orders for surgery
  • List of current medications and allergies
  • Copies of advanced directives or power of attorney

Enter through the main hospital lobby and report to the front desk at your scheduled time. A staff member will escort you to the day surgery unit. Once in the day surgery unit, you'll be prepped and prepared for your surgery or procedure. Our team will ensure you’re comfortable and ready.

Wear loose, comfortable clothing. If you're having an outpatient procedure, make sure to bring someone with you to drive you home afterward.

Family members will be directed to the waiting room during your procedure. Afterward, they’ll have a chance to speak with your surgeon or doctor for an update.

Depending on your procedure, you’ll be taken to one of the following:

  • Recovery room
  • Admitted to a hospital floor
  • Back to day surgery
  • Discharged to go home for recovery
