Insurance acceptedSt. Luke’s Memorial offers an online patient record request tool, which is easy to use and free. If you prefer to pick up your medical records in person, you must complete a medical authorization form and bring it or mail it to St. Luke's Memorial – Livingston.
Birth certificates
A birth certificate is issued for all infants born at St. Luke's Health – Memorial and sent to the State of Texas.
If the father’s name will be included on the birth certificate, an Acknowledgement of Paternity (AOP) form must be completed during the baby’s hospital stay or within the first 3 days of life. AOP forms can be completed Monday through Friday, between 8 a.m. and 3 p.m.

You can preregister at the hospital admissions desk or by phone. During preadmission, you’ll need to provide the following:
Please schedule a preadmission appointment for required tests (like blood work, EKG or X-rays) and to meet with a nurse for health history, surgical consent and pre/post-op education. You may also consult with an anesthesiologist.
Bring the following to your appointment:
Enter through the main hospital lobby and report to the front desk at your scheduled time. A staff member will escort you to the day surgery unit. Once in the day surgery unit, you'll be prepped and prepared for your surgery or procedure. Our team will ensure you’re comfortable and ready.
Wear loose, comfortable clothing. If you're having an outpatient procedure, make sure to bring someone with you to drive you home afterward.
Family members will be directed to the waiting room during your procedure. Afterward, they’ll have a chance to speak with your surgeon or doctor for an update.
