Medical records in Houston

We offer an online patient record request tool, which is easy to use and free. This service is offered to patients of:

You'll need to verify your identity by uploading a photo of your driver's license, which you can take with your phone or computer.

If you’d like to pick up your records in person, please complete the Authorization for Use or Disclosure of/Access to Protected Health Information form and bring it to the medical records department at any St. Luke’s Health hospital.

By mail

You can mail your medical authorization form to the appropriate facility. There may be a fee for accessing your records. Call our medical records department at (832) 355-2077 and speak with one of our representatives before submitting your form.
