If you’d like to pick up your records in person, please complete the Authorization for Use or Disclosure of/Access to Protected Health Information form and bring it to the medical records department at any St. Luke’s Health hospital.

You can mail your medical authorization form to the appropriate facility. There may be a fee for accessing your records. Call St. Luke’s Health’s medical records department at (832) 355-2077 and speak with one of our representatives before submitting your form.
