We’ve launched the CommonSpirit Patient Portal to streamline how you access your health information and manage your care with St. Luke’s Health and Baylor St. Luke’s Medical Group.

You can expect the same secure, convenient access to your health information with a refreshed look that reflects the association of St. Luke’s Health and Baylor St. Luke’s Medical Group with CommonSpirit Health.

Our goal is to make this transition as seamless as possible for you. Please find answers to commonly asked questions below.

After downloading: Using the portal / app

I have downloaded the CommonSpirit app, do I need to do anything else?

No. Once you’ve downloaded the CommonSpirit App and logged in successfully using your existing MyChart username and password, you’re all set. We encourage you to explore its features and start using them to manage your health.

What can I do in the portal?

The CommonSpirit Patient Portal is a great tool to help you manage your health - anytime, anywhere. You can use the portal to:

  • Schedule and manage appointments with your established providers 

  • Complete pre-check-in tasks

  • Carry your health history and information in your pocket 

  • View your recent medical records and test results

  • Communicate securely with your care team

  • Request prescription refills

  • Pay your bills online

  • Access your After-Visit Summary (AVS)

  • Complete post-visit questionnaires

  • Easily utilize the extended Find a Doctor and Find a Location functionality to support your care needs

The CommonSpirit Patient Portal is your one-stop shop for any care you receive from St. Luke’s Health (CommonSpirit) physicians, clinics or hospitals.

And now you can easily connect to our nation-wide network of care, wherever you are. Your records and information travel with you to any CommonSpirit location across the nation.

My health information appears incomplete. Why can’t I see my previous medical records?

Some older records may not appear right away. Your most recent health information has been transferred, and additional records will continue to transfer over time. If you don’t see something you’re looking for, St. Luke’s Health MyChart will remain available with your health information.

What if I change doctors or visit a different CommonSpirit location?

The app follows you across the CommonSpirit network. Your records, appointments and messages stay with you, even if you visit providers at different CommonSpirit hospitals or clinics.

Can I use both MyChart and the CommonSpirit app to access my health information?

Yes, the St. Luke’s Health MyChart is not going away. The CommonSpirit Patient Portal is an enhanced version of MyChart designed to make it easier to stay connected and communicate with your care team.

For a seamless experience and access to enhanced features, we recommend removing the older app and using the CommonSpirit Mobile App. 

Checking in for appointments
Can I check in for appointments through the patient portal or app?

Yes, you can complete check-in steps through the patient portal or app. 

When accessing the patient portal via mobile app or web browser, you'll see an option to check in for upcoming appointments. You might also get notifications about pre-check-in tasks through the portal or text, based on your settings. Note that you may still be asked to check in at the appointment. 

What kind of information will I need to provide during the pre-check-in process?

During the pre-check-in process, you may be asked to verify your insurance info, update your medication list, complete health questionnaires and review and sign consent forms. Please note that you may still be asked to check in when arriving for an appointment. 

Is it required to complete pre-check-in tasks before my appointment?

Completing pre-check-in tasks before your appointment isn't mandatory, but it saves time and helps us prepare for your visit. If you can't do them in advance, you can finish these steps when you arrive.  

What happens when I arrive for my appointment?

Feel free to check in at the front desk or use one of our easy-to-use kiosks. Our team members will be happy to help you with the check-in process.

After-Visit Summary
How do I access my After-Visit Summary (AVS)?

You will be notified when your After-Visit Summary (AVS) is ready in the CommonSpirit Patient Portal and app. To view it, log in and select "View After Visit Summary" under your recent appointment. If not visible immediately, check back later; if still unavailable, contact your provider's office.

What if I have questions about the information in my After-Visit Summary (AVS)? 

Non-urgent medical questions can be submitted through the patient portal or by contacting your clinic or care facility directly. If you need an immediate response or have an urgent request, please call your clinic or care facility.

Will I receive any questionnaires after my visit?

You may receive a notification about a new questionnaire related to your visit or hospital stay in the patient portal and mobile app. Please complete the questionnaire to help us improve our services.

Provider messaging
Where can I message my provider? 

You can message your providers directly through the CommonSpirit App.

Prescriptions
Can I use the portal to view my prescribed medications? 

Yes. Your prescribed medications from St. Luke’s Health have been automatically transferred to the CommonSpirit Patient Portal. 

Can I request a refill of my prescription through the portal or mobile app? 

When your prescriptions are visible in the CommonSpirit Patient Portal, you can request refills there. Until then, contact your pharmacy for refills.

Billing
How do I pay an outstanding bill on the CommonSpirit Patient Portal?

To pay a bill, please visit the “Billing” tab on the CommonSpirit portal or  app menu.

What is the CommonSpirit Patient Portal?

The CommonSpirit Patient Portal is our , secure online tool that lets you take charge of your health — anytime, anywhere. You can view test results, message your care team, pay bills and more. You can access the portal from your computer or download the CommonSpirit App on your phone from the App Store (iPhone)or Google Play (Android).

Why did St. Luke’s Health make this change?

Helping our patients manage their health has never been easier with our  CommonSpirit Patient Portal. This portal is a tool designed to simplify and improve your experience by providing a single, secure place to manage your health online or from your mobile device. 

Do I need to create a new account?

No. If you have an existing MyChart account, you can use the same username and password when logging into the Commonspirit Patient Portal. There’s no need to set up a new account. If this is your first time using our portal, access the Patient Portal on our website or through the app and click “Sign Up Now” to create an account. The CommonSpirit Patient Portal will guide you through the process of setting up your username and password.

It's important to know that the St. Luke’s Health MyChart is not going away. The CommonSpirit Patient Portal is an enhanced version designed to make it easier to stay connected and interact with your care team.

What does this mean for my medical records and health information?

Your medical history, records and messages with St. Luke’s Health have been securely transferred to the CommonSpirit Patient Portal. 

Is the portal safe and secure?

Yes. We take your privacy very seriously. The CommonSpirit Patient Portal uses industry-standard technology and security measures to protect your personal health information. Only you and your authorized caregivers can access your account, and all data is encrypted to prevent unauthorized access. 

Why does the portal look different?

It’s important to know that St. Luke’s Health is part of CommonSpirit. The refreshed design reflects our connection to CommonSpirit.

Where can I download the mobile app?

You can download the CommonSpirit App for free from the App Store (iPhone) or on Google Play (Android). Search for “CommonSpirit – AR/GA/KY/TN/TX” to find the app quickly.

Has my provider or care team changed?

No. Your care team remains the same St. Luke’s Health care team you know and trust. The portal is simply a better way to stay connected with that team.

I’m having trouble logging in. What should I do?

Try the following:

  • Double-check your username and password

  • Click “Forgot Password?” to reset your login

  • Contact our support team if you’re still having trouble

Can I continue using the MyChart app? 

Yes, the St. Luke’s Health MyChart is not going away. The CommonSpirit Patient Portal is an enhanced version designed to make it easier to stay connected and interact with your care team.

For a more seamless experience and access to enhanced features, we recommend switching to the CommonSpirit Mobile App. 

To uninstall your older app:

To remove on iPhone:

  • Find the MyChart icon on your home screen

  • Tap and hold the app icon until a menu appears

  • Select Remove App

  • Tap Delete App to confirm.

To remove on Android: 

  • Open your app drawer and locate MyChart

  • Tap and hold the app icon

  • Drag it to the Uninstall option that appears or tap Uninstall in the menu

  • Confirm the uninstallation

Why am I receiving messages and notifications from CommonSpirit Health?

To ensure a unified experience across all of your care, the patient portal will reflect the CommonSpirit brand. This means some of your messages and notifications, such as important reminders and test results, may now come from CommonSpirit. Rest assured, these messages are from the same physicians and caregivers you’ve always relied on.

What if I don't have a computer or smartphone, or I'm not comfortable using technology?

We understand that not everyone has access to technology or feels comfortable using it. You can still access your health information and manage your care by contacting your provider's office directly. You can request a printed copy of your AVS (After Visit Summary) and complete any necessary questionnaires over the phone. We are committed to ensuring that everyone has access to the care they need, regardless of their technological abilities and preferences.

We know the health care environment is complicated — but your experience with your health information doesn't have to be. This tool is designed to simplify and improve your experience through one single, secure destination to manage your health online. This platform is designed to be intuitive and user-friendly, making it easier to view test results, communicate with your care team and more.

Do I need to create a new account?

Yes. You will need to register for a new account in the CommonSpirit Patient Portal, even if you currently use MyChart. Please sign up for a new account on the CommonSpirit Patient Portal. You will be guided through the process of setting up your username and password.

If you have used MyChart before, you will still have access to your past health information.

Please note: if you activated your CommonSpirit Patient Portal as a patient at Baylor St. Luke’s Medical Center, Lakeside, Springwoods, Sugarland, Vintage, Woodlands, or one of our freestanding emergency departments, no action is needed. Your health information for any CommonSpirit location across the greater Houston area is now available in one place. 

What does this mean for my medical records and health information?

We are committed to ensuring you have continued access to your health information as we make this transition to the Common Spirit Patient Portal. Here’s what you need to know:

  • Your health information from St. Luke’s Health and Baylor St. Luke’s Medical Group doctors’ offices and clinic visits has transferred to the CommonSpirit Patient Portal, and is available once you create an account. 

  • Your doctors still have full access to your records, and you can always call your doctor’s office to manage appointments, get test results or ask questions.

If I am using the St. Luke’s Health and Baylor St. Luke’s Medical Group MyChart portal today, do I need to create a new account for the new portal?

Yes. You will need to register for a new account in the CommonSpirit Patient Portal. You can access your health information from St. Luke’s Health and Baylor St. Luke’s Medical Group doctors’ offices and clinic visits in the CommonSpirit Patient Portal once you create an account.      

How is the CommonSpirit Patient Portal different from the MyChart portal I use today?

The CommonSpirit Patient Portal offers the same secure, convenient access to your health information that you’re used to. The portal will have a refreshed look that reflects the association that St. Luke’s Health and Baylor St. Luke’s Medical Group have with CommonSpirit. 

Will my health information from my St. Luke’s Health or Baylor St. Luke’s Medical Group MyChart portal be transferred to the new portal?

Yes. Your medical records have automatically transferred to the CommonSpirit Patient Portal and will be available once you create an account. Any new updates or messages from your doctor will appear in the new patient portal.

Can I continue using the standard MyChart app?

Yes. You still have access to view your past health information in your current MyChart. However, for a more seamless experience, we recommend you remove the older app and install the CommonSpirit Mobile App.

How do I uninstall previous patient portal apps?

To remove on iPhone:

  • Find the current patient portal app icon on your home screen

  • Tap and hold the app icon until a menu appears

  • Select Remove App

  • Tap Delete App to confirm

To remove on Android: 

  • Open your app drawer and locate your current patient portal app  

  • Tap and hold the app icon

  • Drag it to the Uninstall option that appears or tap Uninstall in the menu

  • Confirm the uninstallation

For Current MyChart Portal Users

What happens to my old MyChart account?

You still have access to view your health information in your old MyChart account. Your past health information will be available in the CommonSpirit Patient Portal once you create an account. 

Will I need to set up a new account with the CommonSpirit Patient Portal?

Yes, you will need to make a new account for the CommonSpirit Patient Portal. Your medical records have automatically transferred to the CommonSpirit Patient Portal and will be available once you create an account.

What if I need help managing my care before my next appointment with my provider? 

Your doctor still has full access to your records and health information. In addition, you can always call your doctor’s office to manage appointments, get test results or with questions. For prescription refills, we recommend calling your pharmacy.

Checking in for appointments

Can I check in for appointments through the patient portal or mobile app?

Yes. When you access the patient portal through the mobile app or your web browser, you will see an option to check in for an upcoming appointment. You may also receive notifications about pre-check-in tasks through the patient portal and/or via text message, depending on the communication preferences you have selected in your patient portal settings. Please note that you may still be asked to check in when arriving for an appointment. 

What kind of information will I need to provide during the pre-check-in process?

You may be asked to verify your insurance information, update your medication list, complete health questionnaires and review/sign consent forms during the pre-check-in process. Please note that you may still be asked to check-in when arriving for an appointment. 

Is it required to complete pre-check-in tasks before my appointment?

While it's not required, completing pre-check-in tasks before your appointment will save you time at the office and allow us to better prepare for your visit. If you don't have time to complete pre-check-in tasks before your appointment, don't worry. You can still complete the necessary steps upon arrival for your appointment. 

What happens when I arrive for my appointment?

You can check in at the front desk or use one of our convenient kiosks. Our staff will be available to assist you with the check-in process.

Accessing my After-Visit Summary

How do I access my After-Visit Summary (AVS) after my appointment?

You will receive a notification when your After-Visit Summary is available in the CommonSpirit Patient Portal and Mobile App. Log in and select "View After Visit Summary" under your most recent appointment. If you don't see your AVS in the portal right after your visit, please check back later as it may take some time for the AVS to be processed and uploaded. If it's still not available after a reasonable amount of time, contact your provider's office.

What if I have questions about the information in my After-Visit Summary (AVS)? 

Non-urgent medical questions may be submitted through the patient portal or by contacting your clinic or care facility directly. If you need an immediate answer or have an urgent request, please call your clinic or care facility.

Will I receive any questionnaires after my visit?

You may receive a notification about a new questionnaire related to your visit or hospital stay in the patient portal and mobile app. Please complete the questionnaire to help us improve our services

Messaging my provider

When can I message my provider? 

Once you sign up for the CommonSpirit Patient Portal, you will be able to message your providers directly. To ensure you are able to receive messages electronically, please complete the pre-registration form at the time of your scheduled appointment.

Managing my prescriptions

Can I use the portal to view my prescribed medications

Yes. Medications from St. Luke’s Health and Baylor St. Luke’s Medical Group will be automatically transferred to the CommonSpirit Patient Portal, once you create an account. 

Can I request a refill of my prescription through the portal or mobile app? 

Yes. Once your prescriptions appear in the CommonSpirit Patient Portal, you can request refills. In the meantime, please contact your pharmacy to refill a prescription.

Managing a bill      

How do I pay an outstanding bill on the CommonSpirit Patient Portal?

You can log into your CommonSpirit Patient Portal online or through the CommonSpirit Mobile App. Follow the online instructions for paying your bill:

  • Navigate to the View Bill option.

  • Once you click on View/Pay Bill, your account information will be displayed, including open visits with balances, total balance owed and itemized and monthly statements. 

Notice: "Find a Doctor" is provided by CommonSpirit Health as a convenience to you. The information on each participating doctor is submitted to CommonSpirit Health by each doctor and is not independently verified by CommonSpirit Health. Each doctor is solely responsible for the completeness and accuracy of the information listed about him or her. CommonSpirit Health is not responsible for any loss or damages caused by your reliance on the doctor information contained on Find a Doctor. You should telephone the doctor's office in advance to verify the accuracy of the information.
