Geriatric care focuses on the health and well-being of older adults, addressing medical, functional and emotional needs that may change with aging. This type of care emphasizes prevention, chronic condition management and maintaining independence and quality of life. Geriatric care providers work closely with patients and families to create personalized care plans that support safe, respectful and coordinated care as people age.
At Dignity Health Connected Living, we offer several services to help older adults live more independently:
Adult day services
Care management
Food assistance
Transportation
Connected Living offers Shasta County residents a variety of ways to access healthy meals, including food bank programs for families and seniors, government commodity distributions, home-delivered Meals on Wheels, and congregate meals at senior dining centers.
Our programs are designed to meet diverse needs, offering options like on-site pick-up, convenient delivery or social meals. Most are easy to access with a simple registration process. Please email our team with questions or for help finding the right option for you.
Geriatric care may be helpful when an older adult is managing several medical conditions, experiencing memory changes, frequent falls or difficulty with daily activities. It can also provide support after a hospital stay or during changes in health or living situations. Early geriatric care can improve coordination, safety and long-term health outcomes.
Geriatric care recognizes the important role families and caregivers play. Our care teams often provide education, guidance and care planning support to help caregivers understand conditions, treatments and next steps. This collaborative approach helps ensure care decisions reflect the patient’s needs, goals and dignity.
We also have a monthly group that supports caregivers with resources, education and peer connection. It’s open to anyone caring for someone with a disability or health condition. Topics and speakers change each month, with time to share and connect. Free respite care is available if needed.
This group meets the third Wednesday, 5:30–6:30 p.m. RSVP by the Monday before at (530) 226-3000.
