Connect to a Community Health Worker
A Community Health Worker (CHW) works closely with you to understand your unique needs, whether that's finding health insurance, a ride to a doctor's appointment, or a safer, more secure place to live.

The CHW helps them find these resources and stays in touch until everything is taken care of. CHWs also teach people about staying healthy. The best part? It’s all free for the person getting help. CHWs are paid through healthcare organizations who understand that if a person is healthy, they are more likely to stay healthy and live well in their community.
Behind the scenes, the HUB, a non-profit organization in the community, keeps everything running smoothly. It handles the money, technology, training, and data. This lets CHWs focus on helping people. The program is established in over 35 communities in the U.S., including these CommonSpirit regions: Arkansas, Arizona, California, Kansas, Nebraska, Nevada, Tennessee, and Texas.
The Real-Life Impact in the Community
In Clark County, Nevada, we collaborated with local partners to listen to community needs and, in response, launched a Pathways Community HUB (led by Comagine Health) to help pregnant moms and their babies. With support from local grants, CHWs helped pregnant women get the care they needed. The program's clients had healtheir pregnancies, resulting in fewer babies born early or with low birth weight in the same area compared to the county averages.
A young pregnant woman in Texas needed help getting Medicaid and finding a place to live. She only spoke Spanish, so it was hard for her to apply for health care insurance. A CHW helped her fill out the Medicaid forms and she was approved. She could now go to her doctor appointments and take care of her baby. The CHW also found help to pay her rent and utility bills. In December 2023, she had a healthy baby boy. Her husband got a new job, and things are going well.
Thanks to the CHW, she was able to:
- Get health insurance
- Sign up for WIC and SNAP (to get food)
- Get baby items like a car seat, diapers, and clothes
- Get help with rent and utility bills
- Have a healthy baby and care after birth

Our partnership with PCHI provided care outside of our clinics and hospitals, to serve and support communities. We believe everyone should have what they need to live a healthy life. Here's what we’re working on:
- Access: Making sure everyone can find help easily
- Availability: Making sure there are enough services and helpers
- Integration: Including community workers as part of the care team
- Sustainability: Finding long-term support for services
- Quality: Listening to the community when designing care
