Dr. Austin Hazlewood Joins CHI Memorial Family Practice Associates

CHATTANOOGA, TN – CHI Memorial Medical Group is pleased to welcome Austin Hazlewood, DO, to CHI Memorial Family Practice Associates - Cleveland.  Dr. Hazlewood earned his medical degree from Debusk College of Osteopathic Medicine in Harrogate, Tennessee, and completed a residency in family medicine at the University of Louisville in Glasgow, Kentucky.

Dr. Hazlewood joins Jamie Bird, FNP-C, and Chandler Chastain, FNP-BC, at CHI Memorial Family Practice Associates - Cleveland.  The practice emphasizes disease prevention by focusing on treating the whole person – from childhood to adulthood - with a goal of providing comfortable, convenient, and prompt care to help you get well and stay well.  Dr. Hazlewood and Chandler Chastain welcome new patients.
