Accessing your medical records

Your medical records contain important information about your health history, diagnoses, treatments and medications. They help us provide you with the best possible care. Whether you need your records for personal use or to share them with another health care provider, we are here to help.

You have the right to get a copy of your medical records. You can also ask for your records to be sent to another person or organization. This could be another health care provider, a family member or a legal representative.

To request your records, complete an authorization for the release of health information form. You can submit the form by mail, fax or in person at one of our medical records departments. Clearly state who can receive your records and for what purpose.

For personal use or third-party requests, a processing fee applies based on processing time, media type, delivery method and California sales tax. VRC will provide advance notice of charges, and payment is due before delivery. No fee applies for records sent directly to another health care provider. Records can be delivered via VRC’s secure electronic portal, USPS mail or other carriers.

You can view a summary of your medical records anytime through our secure online patient portal. This includes information like your lab results, medication history and upcoming appointments. It’s a convenient way to stay informed about your health.
