EpicCare Link is a web-based application from Epic designed to facilitate secure communication and coordination between health care providers and their community affiliates. While it is not a full electronic medical record (EMR) system, it serves as a read-only platform with service-oriented features to support patient care coordination with community practices.
Through EpicCare Link, community providers, such as referring physicians, community clinicians and care team members, gain secure access to specific patient information stored in CHI Memorial’s Epic data repository. This access is intended for individuals outside of CHI Memorial who need to review clinical and administrative details about patients treated at our organization.
Common users of EpicCare Link include affiliate clinical staff, billing personnel, staff from long-term care facilities and employees of correctional facilities.

Step 1: The facility or practice must first complete a new EHR Agreement and enroll the practice using the Enroll Practice link. Each EpicCare Link user must be associated with an enrolled health care organization and must still complete an Individual User Request.
Step 2: A site administrator must be assigned to the practice. Use the Site Administrator Request link to request enrollment as a site administrator for a health care organization. Site administrators are responsible for resetting user passwords and validating user access every 90 days.
Step 3: Once the practice has been enrolled, individual practice users may request access using the Individual User Request link. Each EpicCare Link user must submit a separate request for access and be associated with a health care organization.
Step 4: End user completes their portion and saves the form. This will populate the site administrator's email address into a link. The end user clicks to send the email.
Step 4: The practice manager completes and submits the request.
