Improving population health

Healthier people. Stronger communities. A health care system better matched to our ideals and the drivers of poor health. CommonSpirit Health has an ambitious agenda as we serve more than 2.7 million people under value-based agreements across our communities.

We serve a broad mix of people in diverse communities across the country and seek to:

  • Develop key infrastructure to enable and accelerate CommonSpirit’s population health capabilities
  • Drive excellence in value-based care
  • Meaningfully, measurably improve health and promote health equity
  • Position CommonSpirit Health as a nationally-recognized source of population health thought leadership and advocacy
Our direction for population health

CommonSpirit Health will be the nation’s leading value-based health system, illuminating a path for the transformation of American health care premised on a commitment to equity, population health, financial stewardship, and a prioritization of the vulnerable.

Programs and initiatives

From promoting health equity by leveraging data and technology to addressing major health care issues such as opioid use disorder and Hepatitis C, CommonSpirit Health is committed to health care innovation through our various initiatives.

As we strive to improve the health and wellbeing of the various populations with serve, particularly for the most vulnerable, we are dedicated to finding new and equitable approaches and strategies that reach all patients and remove barriers to care.
