A New, Less-Invasive Treatment for Male Infertility

Dr. Taylor Kohn has pioneered an ultrasound technique for sperm retrieval. See how this innovative technology is transforming fertility care.

“It is very common for men to feel embarrassed or out of place in fertility care. My role is to make this process as clear and straightforward as possible”

For couples navigating infertility, a diagnosis of azoospermia — the absence of sperm in the semen — can feel insurmountable. While this diagnosis can be overwhelming, it is important to know that you are not alone. Infertility affects approximately 15% of couples worldwide, while  male health factors contribute to about 50% of these cases.

For years, the standard path for men with azoospermia has involved surgical sperm retrieval. While these procedures have helped many, they are often perceived as invasive, complex and emotionally taxing.

However, the field of male fertility is evolving. Taylor Kohn, MD, a urologist at Baylor College of Medicine and St. Luke’s Health – Sugar Land Hospital, is leading that change by using high-frequency ultrasound to map the testicles. This method makes sperm retrieval more precise and less invasive.

Understanding Your Options

According to the American Urological Association (AUA), when a semen analysis shows no sperm, the first step is to work with a urologist to determine the cause. Specialists categorize these cases as either obstructive, which is a blockage, or non-obstructive, which is a production issue. Identifying the root cause is the key to finding the right treatment path.

Common testing and treatment pathways often include:

  • Diagnostic testing: This includes blood hormone and genetic testing to identify the root cause of azoospermia. Many cases can be effectively managed with targeted medications, hormone therapies or lifestyle adjustments — such as weight management or dietary changes — to improve sperm health before considering surgery.

  • Non-surgical methods: For men with specific conditions that prevent ejaculation, treatments like penile vibratory stimulation (PVS) or electroejaculation (EEJ) may be an option.

  • Surgical sperm retrieval: When non-surgical methods aren't applicable, procedures like testicular sperm aspiration (TESA) or microsurgical testicular sperm extraction (Micro-TESE) are used to collect sperm for use with in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI).

A Technological Leap: Ultrasound-Guided Retrieval

While traditional microsurgical techniques like Micro-TESE remain the standard, Dr. Kohn is advancing care by adding a diagnostic layer of high-frequency ultrasound — similar to the technology used in dermatology for detailed imaging.

“Traditionally, surgeons have relied on their eyes to find sperm during a micro-dissection,” Dr. Kohn explains. “This new method makes the process much more efficient. By using high-frequency ultrasound to map the testicle beforehand, we can identify specific areas where sperm production is more likely. It acts like a GPS for the surgery, allowing us to be significantly less invasive while keeping our success rates high.”

Dr. Kohn has successfully used this technology with over 120 patients, predicting the localized presence or absence of sperm within the testicle with 93% accuracy. While success rates for sperm retrieval vary depending on the underlying cause, modern techniques continue to push the boundaries of what is possible.

Navigating the Emotional Side of Care

Technological advancements provide a clear path forward, but  the emotional journey remains important. It is common for a diagnosis like this to feel isolating. Many men feel like they need to navigate this journey in silence or that fertility struggles are a private burden they must handle on their own. If you have felt sidelined or uncomfortable, please know that those feelings are valid and you don't have to face them alone.

Dr. Kohn emphasizes that your comfort is a priority throughout the treatment process.

“It is very common for men to feel embarrassed or out of place in fertility care,” Dr. Kohn says. “My role is to make this process as clear and straightforward as possible. There are many avenues for care — from hormonal management and lifestyle changes to advanced surgical techniques. When we demystify the process and ensure you feel comfortable with your care team, it changes the entire experience. My goal is to make sure you have the information you need to make the right decisions for your family.”
