Follow these helpful tips during your next appointment:
Bring a complete list of your medications, including over-the-counter drugs, vitamins and herbal supplements, along with their dosages and directions.
Please arrive 30 minutes early and no more than 5 minutes late to ensure timely check-in and minimize wait times. Our goal is to have you in the exam room at your appointment time.
Write down any questions or concerns before your appointment, and be sure to address them during your visit.
Only urgent issues will be addressed after hours. For non-emergent concerns, please schedule an appointment. Prescription refills are limited outside regular office hours.
Inform us no later than 24 hours in advance if you need to cancel your appointment.
Don’t forget to schedule your follow-up appointment before leaving the office to ensure timely and consistent care.
If you have lab work or testing done during your visit, you should receive your results in the mail within 10-14 days. If you have questions about your results, please schedule an appointment to discuss them. We kindly ask that you refrain from calling the office unless you have not received your results after 15 business days.

We understand that visiting the clinic, leaving messages, waiting on hold or updating personal health information isn’t always convenient. That’s why we’re proud to offer secure online access to your health information, making it easier and more convenient to manage your care.
We accept most major insurance plans. Check your policy to confirm coverage so we can submit the claim correctly. All insurance copays, coinsurance and deductibles are due at the time of service.

We respect your time and strive to keep wait times to a minimum. To assist us in providing timely care, please notify us no later than 24 hours in advance if you need to cancel your appointment. Missed appointments without notice may result in a fee.
Please inform your provider about any medications requiring refills during your scheduled appointment. All medication refills are subject to provider approval. Refills for controlled substances require an appointment every 90 days and cannot be filled outside of a scheduled visit. After-hours refills (excluding controlled substances) are limited to a 3-day supply. To ensure uninterrupted access to your medications, please schedule an appointment in advance to discuss and obtain refills.
When calling for medication refills, delayed lab or test results or non-urgent health concerns, please select your nurse’s line. Speak clearly and provide the following information:
Full name: Include spelling.
Phone number and date of birth.
Reason for call: If requesting a prescription refill, provide the medication name, dosage, pharmacy name and phone number. For non-prescription issues, include symptoms, when they started and any home remedies you’ve tried.

Your health information is confidential, except as required by law, insurance guidelines or in emergencies.
Patient Responsibilities
Provide accurate information about your health, medications, insurance, contact details and any relevant medical history or conditions.
Keep your personal information updated, including insurance cards, driver’s license, contact information and legal name changes. This ensures accurate appointment reminders and lab results.
Bring necessary forms, such as prior authorization, FMLA, disability or medical supply forms, to your appointment. Please allow 72 hours for completion after your visit. For prior authorization denials, contact your insurance for further details.
