At CHI Memorial Infectious Disease Associates, we provide expert, team-based care to address the complexity of infectious illnesses. Your doctor collaborates with skilled providers to tailor care to your needs, focusing on improving your overall health from every angle. Our specialists diagnose and treat challenging illnesses caused by bacteria, fungi, and viruses, including HIV/AIDS, tuberculosis and sexually transmitted infections. They are nationally and locally recognized, earning the Center of Excellence for HIV/AIDS designation from the Tennessee Department of Health and the U.S. Department of Health and Human Services. Follow these helpful tips during your next appointment:

  • Bring a complete list of your medications, including over-the-counter drugs, vitamins and herbal supplements, along with their dosages and directions.

  • Please arrive 30 minutes early and no more than 10 minutes late to ensure timely check-in and minimize wait times. Our goal is to have you in  the exam room at your appointment time.

  • Write down any questions or concerns before your appointment, and be sure to address them during your visit.

  • Don’t forget to schedule your follow-up appointment before leaving the office to ensure timely and consistent care.

Our medical case manager works closely with patients receiving benefits through the Ryan White Program, which provides access to critical care and resources, regardless of your insurance status. Our dedicated medical case manager and infectious disease physicians work together to ensure seamless coordination of your care, connecting you with the services and offering assistance with:

  • Accessing health-related services not provided by our center, such as primary care, dental care, nutrition assistance or mental health counseling

  • Applying for medication assistance programs and health insurance enrollment

  • Determining Ryan White coverage eligibility

  • Maintaining your care plan

  • Understanding insurance premiums, deductibles and copayments

We may also assist with referrals to:

  • Alcohol or drug rehabilitation programs

  • Emergency financial assistance

  • Housing assistance

  • Job training

  • Psychosocial services

  • Vision assistance

We accept most major insurance plans. Check your policy to confirm coverage so we can submit the claim correctly. All insurance copays, coinsurance and deductibles are due at the time of service.

We respect your time and strive to keep wait times to a minimum. To assist us in providing timely care, please notify us no later than 48 hours in advance if you need to cancel your appointment. Missed appointments without notice may result in a fee.

When calling for medication refills, delayed lab or test results or non-urgent health concerns, please select your nurse’s line. Speak clearly and provide the following information:

  • Full name: Include spelling.

  • Phone number and date of birth.

  • Reason for call: If requesting a prescription refill, provide the medication name, dosage, pharmacy name and phone number. For non-prescription issues, include symptoms, when they started and any home remedies you’ve tried.
