Dignity Health makes it easy for you to track essential health information, keeping you informed and in control of your well-being. With My Portal, you get secure online access to your health information anytime, anywhere.
There are a few different ways to access your My Portal via Commonspirit.org.
Accessing your portal based on the area you receive care
Patients in Nevada and the following regions of California (Bay Area, Central Coast, Ventura County and Southern California) can access My Portal by clicking here.
Patients in Arizona (excluding Yavapai Regional Medical Center patients) and the following regions of California (Greater Sacramento, Central Valley and Northern California) can access My Portal by clicking here.
Yavapai Regional Medical Center patients can access YRMC Care Connect by clicking here.
Accessing your portal with our Portal Access Wizard
You can also access your My Portal account by clicking on Patient Portal in the top of the website and look up your portal by entering the provider or location where you receive care.
To begin, click the Patient Portal link circled below.
Then enter the name of the provider or location where you received care.
Once you find your provider, click on the “Sign in” link below.

My Portal access helps you stay connected before, during and after care. During your visit or after scheduling an appointment, you may receive an email invitation from Dignity Health (no-reply@commonspirit.org). Follow the instructions in the email to create your account, then sign in using your email and password. If you already have a My Portal account, you may also download the My Portal mobile app from the Apple App Store or Google Play.

Can I create an account for a family member?
Yes. Children under 12 and dependent adults can have a My Portal account with an adult as the proxy. Adolescents 12-17 cannot have an account. This is because teens have legal privacy rights for certain types of care, and our system isn’t designed to separate what parents and teens can see. For dependent adults, a signed release or Medical Durable Power of Attorney is required.Who can access my account and medical information?
You and authorized Dignity Health staff can view your medical records. You may also give a trusted person, such as a family member or caregiver, proxy access to help manage your care. You can change or remove proxy access at any time. You may be able to add or manage proxy access through My Portal. If you don’t see this option, contact Health Information Management or ask your care team for help.- What if my portal invitation is lost or expired?
Invitations expire after 90 days. You can request a new invitation from your Dignity Health facility or call technical support at (844) 274-8497 for help resending a pending or expired patient portal email invitation.
- Why am I getting a “400 Bad Request” error when I try logging into My Portal by Dignity Health?
My Portal uses different access links based on your care location. This error typically occurs when you try to access My Portal using a link that does not match the region where you receive care. If you select the incorrect regional link, the login attempt will fail and may display a “400 Bad Request” error.
To resolve this, please use the correct link based on where you receive care:
- Patients in Nevada and select regions of California (Bay Area, Central Coast, Ventura County and Southern California) can access My Portal by clicking here.
- Patients in Arizona (excluding Yavapai Regional Medical Center patients) and select the following regions of California (Greater Sacramento, Central Valley and Northern California) can access My Portal by clicking here.
