Accessing your medical records

Your medical records contain important information about your health history, diagnoses, treatments and medications. They help us provide you with the best possible care. Whether you need your records for personal use or to share them with another health care provider, we are here to help.

Request your medical records electronically

St. Joseph Health offers an online patient record request tool, which is easy to use and free. You'll need to verify your identity by uploading a photo of your driver's license, which you can take with your phone or computer. 

 

Other ways to access your records

If you’ve been a patient at one of our hospitals or clinics, you can view a portion of your medical records through one of our patient portals.

To sign up for portal access, you’ll need your medical record number (MRN), which is listed on your discharge paperwork.

  • Hospital patients: If you don’t have your MRN, call our Health Information Management Office at (979) 776-2908.

  • Clinic patients: For help accessing your records, call our Healthcare Referral and Coordination line at (979) 774-2121.
