Accessing your medical records

Medical records are a key part of your health history and include visit summaries, test results, doctor’s notes and treatment reports. You can request copies for personal use or to share with another health care provider. 

Request your records electronically

Baylor St. Luke’s Medical Center offers an online patient record request tool, which is easy to use and free. You'll need to verify your identity by uploading a photo of your driver's license, which you can take with your phone or computer. 

Other ways to request your medical records

If you prefer to get your records in person or by mail, please complete the Authorization for Use or Disclosure of/Access to Protected Health Information to authorize disclosure of records to another person or organization.

By mail

You may mail in or bring your completed form to our health information management office.
