CommonSpirit Medical Records

To obtain medical records from a hospital or hospital-based department, please download and mail the completed hospital authorization form and a copy of a photo ID to the appropriate facility. If requesting records from multiples facilities, list each facility on one form. You may also request your records electronically using our convenient online tool.

To obtain records from a CommonSpirit Medical Group physician practice, download the Medical Group authorization form and mail the completed form to the practice’s mailing address. Contact the practice directly to confirm the address before sending.

There is no charge for records sent directly to a provider or facility for continuing care. Patients are entitled to one free personal copy of their medical records under Kentucky law (Kentucky Revised Statutes Section 422.317). Standard fees apply for additional copies:

• Electronically maintained records (delivered electronically): $6.50

• Electronically maintained medical records combined with paper records (delivered electronically): $6.50 (for the electronic portion) + $0.07 per page (for the paper portion)

• Electronically maintained records (delivered on paper): $0.90 (labor) + $0.05 per page (paper/toner)

• Paper records (delivered electronically): $0.07 per page

• Paper records (delivered on paper): $0.12 per page
