During your first weeks at home, our team supports you with follow-up visits, medication guidance, symptom monitoring and personalized education. 

We work closely with your hospital and primary care doctors. This coordination helps reduce complications and the chance of a return to the hospital. We also use convenient tools, such as at-home tracking devices, to make your return to daily life easier and more comfortable.

You will receive an automated telephone call or text message to remind you of your appointments. Please arrive 15–30 minutes early to allow time for parking, check-in and filling out patient forms. Bring your copayment (if applicable), current medication list, insurance card and photo ID.
