Our team offers many services to help you recover at home after a hospital stay, which can help prevent complications and reduce your chance of returning to the hospital. These services include:

• Education on nutrition, activity and recovery
• Help with discharge instructions

You will receive an automated telephone call or text message to remind you of your appointments. Please arrive 15–30 minutes early to allow time for parking, check-in and filling out patient forms. Bring your copayment (if applicable), current medication list, insurance card and photo ID.
