Accessing your medical records

Your medical records contain important information about your health history, diagnoses, treatments and medications. They help us provide you with the best possible care. Whether you need your records for personal use or to share them with another health care provider, we are here to help.

You have the right to get a copy of your medical records. You can also ask for your records to be sent to another person or organization. This could be another health care provider, a family member or a legal representative.

To request your records, complete an authorization for the release of health information form. For more instructions or details, read the Request for Medical Records Guide Sheet. You can submit the form by mail, fax or in person at one of our medical records departments. Clearly state who can receive your records and for what purpose.

You may also view a portion of your records online through our secure online patient portal. This includes information like your lab results, medication history and upcoming appointments. It’s a convenient way to stay informed about your health.

Requesting medical records

Patient online access form: This form is for patients and legal guardians with legal documentation who want a copy of their own medical records.

Requester submission portal: Use this option if you are a healthcare provider, attorney or requesting medical records for someone else and are not their legal guardian.

Note: You may be asked for additional documents such as a photo ID, birth certificate or power of attorney. You will receive an update or response from Sharecare within 3 business days. For questions, please call (602) 246-3398.

Our medical records department provides support to all patient care areas within the hospital, our urgent care facilities and laboratory. When you receive care at Dignity Health Yavapai Regional Medical Center in Prescott or Prescott Valley or visit your primary or specialty care provider at any Yavapai Regional Medical Group (YRMG) clinic, important information is being added to your medical record. We also assist with information for birth and death certificates and help make changes to your health records.

Your health care team has access to this important information, and with YRMC CareConnect, you have access to much of the same information anytime, anywhere you have an Internet connection.

Log in to YRMC CareConnect to access your health information or sign up for portal access. You may self-enroll using the link below or sign up during your YRMG clinic or Dignity Health Yavapai Regional Medical Center visit.
