If you’d like to pick up your records in person, please complete the Authorization for Use or Disclosure of/Access to Protected Health Information form and bring it to the medical records department at any St. Luke’s Memorial hospital.
You can mail your medical authorization form to the appropriate facility. There may be a fee for accessing your records. Call the medical records department at any St. Luke’s Memorial hospital before submitting your form.

A birth certificate is issued for all infants born at St. Luke's Memorial and sent to the State of Texas.
If the father’s name will be included on the birth certificate, an Acknowledgement of Paternity (AOP) form must be completed during the baby’s hospital stay or within the first 3 days of life. AOP forms can be completed Monday through Friday, between 8 a.m. and 3 p.m.
