The Center for Healthier Communities at Dignity Health – Northridge Hospital Medical Center offers a variety of community-based programs to promote healthy behaviors and meet the unique needs of San Fernando and Santa Clarita Valley residents. Our programs help:

Improve young people’s mental health through health education classes
Increase community emergency and disaster readiness

The LEAP-EMDT LA initiative is dedicated to combating elder abuse and providing comprehensive support for vulnerable adults who are:
60 years of age or older
Residents of LA County
Suspected victims of abuse, neglect or financial exploitation
A multidisciplinary team of geriatric and social workers from community, aging and mental health organizations works to:
Review and assess elder abuse cases
Connect older adults with a robust network of essential services
Conduct regular follow-ups to determine needs for ongoing support and ensure access to benefits

The MHAT program is committed to improving outcomes for individuals with mental illness experiencing homelessness or housing instability. We provide virtual and in-person training in both English and Spanish, including:
Mental Health First Aid (youth or adult)
Question, Persuade, Refer (QPR) training
These trainings equip families, caregivers and community-based providers to:
Recognize the signs and symptoms of mental disorders
Safely and effectively respond to people with serious mental illnesses

Blood pressure self-monitoring program
Free incentives to help manage heart health and blood pressure

Monthly check-in calls with a health educator or pharmacist
Social needs screenings and health navigation support

The AVAAS Project is a two-year project funded by CommonSpirit Health’s Mission and Ministry Fund - United Against Violence Program. The AVAAS Project aims to enhance community capacity across all ages within the San Fernando Valley to prevent violence. We will accomplish this goal through the implementation of young adult education, parent and youth education, older adult education, community engagement and stakeholder involvement and participation.
