CommonSpirit Hospitals Continue to Excel in Patient Safety

Eighty-six percent of eligible CommonSpirit hospitals received A or B Leapfrog safety grades, compared to 58% of hospitals nationwide.

CHICAGO – Nov. 13, 2025 – Patient safety throughout CommonSpirit Health, one of the nation’s largest nonprofit health systems, significantly exceeded the national average in Leapfrog’s fall 2025 Hospital Safety Grades, released today.

Leapfrog’s grading system, based on 22 national performance measures, is designed to assess overall hospital safety. Performance is measured by how well hospitals deliver care, patient experience scores, and quality and safety outcomes such as Patient Safety Indicators (PSI)  and hospital acquired infections (HAI). The scores are converted to one of five letter grades, ranging from “A” to “F”.

The hospital safety grades are a transparent way for patients and communities to see how hospitals measure up, benchmarked against national best practices using standardized, publicly available data, empowering them to make informed decisions about their care.

Forty-nine CommonSpirit hospitals earned an A, and two hospitals have earned straight A’s since 2012 — Virginia Mason Medical Center in Seattle, WA, and French Hospital Medical Center in San Luis Obispo, CA. Another 27 CommonSpirit hospitals earned a B, and 18 hospitals moved up a letter grade from the last ranking.

"Our Fall Leapfrog grades reflect CommonSpirit’s unwavering dedication to patient safety and delivering quality care,” said Dr. Phillip Chang, chief medical and quality officer at CommonSpirit Health. “This achievement speaks to our culture of putting safety first, preventing harm, and continuous improvement, reinforcing the core principles that drive our mission. We’re doing well and we want to become even better. We’ll continue to build upon this success through system-wide initiatives to further elevate the care we provide."

The Leapfrog Hospital Safety Grades are just one measure of a comprehensive patient safety and quality program across CommonSpirit that prioritizes and scales best practices across the health system, which has resulted in CommonSpirit receiving several recent accolades, including:
