St. Luke’s Memorial patient portal

We are introducing our new CommonSpirit Health patient portal and app, designed to deliver the same secure, convenient access you're used to, with a refreshed look and a more seamless digital experience.

• Starting November 1, 2025, Baylor St. Luke’s Medical Group clinics will now be included in the CommonSpirit Portal and app.  You may receive texts or emails to help you activate the patient portal or download the CommonSpirit app from Google Play or the Apple App Store. 

• If you are a new MyChart user, our staff can help you activate your MyChart account during check-in.

• To guarantee you access to all features, please update your bookmarks or favorites to our new web address: patientportalsouth.commonspirit.org.

It’s free and available for patients at the Baylor St. Luke’s Medical Groups. St. Luke’s Memorial hospitals will not be transferring to the new portal or app now, but look for more information in 2026.

How to access the new patient portal and app

• Current MyChart users can download the new CommonSpirit - AR/KY/TN/GA/TX app from the Apple App Store and Google Play starting June 28

• Current users can also access the patient portal using the same account information as their MyChart account

• New users can download the app, and our staff can assist with account activation during check-in at your next visit.
