At St. Joseph Health, our mission is to care for everyone in our community. We dedicate significant resources to programs and services that improve access to health care for underserved populations, including those who are uninsured. 

Every three years, our hospitals conduct Community Health Needs Assessments (CHNAs) to better understand and respond to the health concerns that matter most to our community. We use data and community feedback to identify and prioritize key health challenges and develop and implement plans for community programs and other meaningful investments in health and well-being.

Through strong partnerships with local organizations and communities, we remain dedicated to providing care that truly reflects your needs.

    If you have unresolved quality or safety concerns about the care you received at one of our hospitals, we encourage you to contact the hospital’s executive offices.     In times of serious illness or injury, you may be unable to make your own medical decisions. To ensure your wishes are respected, you can create advance directives. These legal documents allow you to express your preferences regarding medical treatments, including medications, X-rays and lab tests. They also address critical life-sustaining measures such as CPR, mechanical ventilation, dialysis, blood transfusions, and artificial nutrition or hydration. By creating these documents, you can ensure your voice is heard and your values are upheld, even when you cannot speak for yourself.

    Steps to complete your advance directive 

    Complete the appropriate forms according to your wishes. Sign them in the presence of two witnesses or a notary.

    Witness requirements

    Witnesses must be adults who are not:

    • Your designated health care agent

    • Related to you by blood or marriage

    • Entitled to inherit from your will

    • Your caregiver

    • Your doctor or their staff

    • Hospital employees

    Distribution

    • Keep the original in a safe place.

    • Give a copy to your primary care doctor.

    • Bring a copy when admitted to a hospital.

    • Provide a copy to your medical power of attorney agent.

    Texas Health and Human Services provides the following advance directive forms in English and Spanish:

    The DAISY Foundation, founded in 1999 by the Barnes family, honors the memory of J. Patrick Barnes, who died at 33 from complications of idiopathic thrombocytopenia purpura (ITP). Inspired by the exceptional care Patrick received during his eight-week hospital stay, his family created the DAISY Award, which recognizes nurses who significantly impact patients' lives.

    DAISY Award honorees at St. Luke’s Memorial embody our core values:

    • Reverence: Respect for all creation, shaping spirituality and relationships

    • Integrity: Moral wholeness, fidelity and truthfulness

    • Compassion: Solidarity and empathy for others

    • Excellence: Outstanding performance and dedication to being our best.

    Nomination process

    Fill out the nomination form to nominate a nurse who demonstrates clinical expertise, compassionate care and commitment to patient safety, serving as role model in our nursing community.

      The BEE Award recognizes non-nursing staff based on five key principles (PETAL: Passion, Excellence, Teamwork, Accountability, and  Leadership).

      Just as a DAISY cannot survive without a BEE, the nurses cannot survive without the outstanding teamwork provided by other health care professionals. 

      Nomination process

      Fill out the nomination form to nominate a team member  (non-nursing staff member) that went above and BEEyond the call of duty to provide excellent patient care or customer service. Share your story and provide as much detail as possible.
