Call 911 right away if symptoms are sudden, severe or include chest pain/pressure, trouble breathing, fainting or symptoms that feel life-threatening.

Shortness of breath, especially with activity or when lying down

Our surgeons see new patients and those looking for a second opinion or a consultation. Your doctor can refer you to our office, or you can self-refer for a consultation. It is important for us to receive your medical records before your first appointment. Once we receive those records, our office will call you to schedule your first appointment. Please bring your completed new patient forms with you to your first appointment. Otherwise, please plan to arrive 30 minutes before your appointment so you can complete the forms. You should also plan to bring your:

Insurance card

The referral process is similar whether a provider refers a patient to our office or someone refers themselves. Simply call our office and ask to speak with the physician referral coordinator or patient referral specialist. They will help gather basic medical information, verify your insurance and help schedule the appointment. They also coordinate communication with your care team to ensure a seamless experience. Most patients are seen within one to two weeks of their referral.
If you are a health care provider, we will provide timely updates regarding your patient’s evaluation and treatment.
