Advance directives give you the opportunity to share your wishes for medical care if you’re unable to speak for yourself. It can outline preferences for life-sustaining treatment, pain control and name someone to make decisions on your behalf.
If you already have an advance directive, please bring a copy with you when you come to the hospital. If you don’t, you can complete a form during your stay or download one in English or Spanish. Our chaplains, case managers, discharge planners and social workers are here to help you through the process.
